If your child has a peanut allergy, you know the weight of it. Every snack, every birthday party, every school lunch comes with a mental checklist. You carry an epinephrine auto-injector everywhere. You read every label twice. And still, the fear of accidental exposure never fully goes away.

Avoidance has been the standard approach for decades — and it's important. But for many families, it's not enough. That's where oral immunotherapy comes in.

At Inspire Allergy & Asthma in Carmel, Indiana, we offer oral immunotherapy (OIT) for peanut allergy as part of our food allergy program. This post walks you through what OIT is, how it works, who it's right for, and what the process looks like at our practice.

What Is Peanut Allergy — and Why Is It So Serious?

Peanut allergy is one of the most common food allergies in children and one of the least likely to be outgrown. Trace amounts of peanut protein can trigger a reaction in highly sensitive individuals. That's what makes peanut allergy so stressful to manage. Cross-contact, hidden ingredients, and unpredictable exposures are a constant concern.

For families in Carmel, Westfield, Fishers, Zionsville, and the greater Indianapolis area, finding an allergist who specializes in food allergy — and who offers treatment beyond avoidance — can be life-changing.

What Is Oral Immunotherapy (OIT)?

Oral immunotherapy is a medically supervised treatment that helps the immune system become less reactive to a food allergen over time. Instead of avoiding the allergen completely, patients consume small, carefully measured amounts of it — starting extremely low and increasing gradually under the guidance of a board-certified allergist.

The goal is not to allow your child to eat peanut butter sandwiches freely (though some patients do reach that level). The primary goal is desensitization: raising the threshold at which a reaction occurs, so that an accidental exposure — a crumb, a trace, a "may contain" item — is far less likely to cause a severe reaction.

For many families, OIT offers something avoidance alone cannot: a proactive layer of protection.

How Does Peanut OIT Work?

Peanut OIT follows a structured, phase-based process that we tailor to each patient. Here is what it generally looks like:

Phase 1: Evaluation and Preparation Before any treatment begins, we conduct a thorough food allergy evaluation. This includes a detailed review of your child's reaction history, prior allergy testing, skin prick testing, and possibly blood testing. We discuss your child's overall health, any other conditions like asthma or eczema, and whether OIT is a safe and appropriate option.

Phase 2: Initial Dosing The first dose is given in our office under close observation. We start with an extremely small amount of peanut protein — far below what would typically cause a reaction — and watch carefully for any response. This visit is longer than a typical appointment, and we do not rush it.

Phase 3: Build-Up Dosing Over the following weeks and months, the dose is increased incrementally. Build-up visits happen in our office, usually every two to four weeks. Between visits, your child takes their current dose at home every day. Consistency is key — daily dosing is what drives the desensitization process.

Phase 4: Maintenance Once your child reaches their target maintenance dose, the build-up phase is complete. They continue taking that dose daily at home. Regular follow-up visits with Dr. Sivam help us monitor progress and fine-tune the plan over time.

Who Is a Good Candidate for Peanut OIT?

OIT is not right for every patient, and part of our job is helping you figure out whether it makes sense for your child. In general, good candidates tend to be:

  • Children and adults with a confirmed IgE-mediated peanut allergy

  • Patients who are medically stable and whose asthma (if present) is well-controlled

  • Families who are ready to commit to daily dosing and regular follow-up visits

  • Those who want more than avoidance — a proactive plan for accidental exposure protection

OIT may not be appropriate for patients with certain medical conditions or for those who are not ready for the commitment the protocol requires. We discuss all of this openly at your first visit.

Why Choose Inspire Allergy & Asthma for Peanut OIT in Carmel?

There are a handful of practices in Indiana offering OIT, but the experience varies significantly. Here is what makes Inspire Allergy & Asthma different:

You work directly with Dr. Sivam — always. As a direct-care practice, we do not route you through call centers or hand you off to a nurse practitioner for your OIT visits. Every appointment, every question, every dose change goes through your allergist.

Visits are longer and unrushed. OIT requires careful monitoring and clear communication. We build that time into every appointment. You will never feel like you are being rushed out the door.

Transparent, upfront pricing. We believe families deserve to understand the cost of OIT before they begin. Our pricing is listed clearly on our website with no surprise bills.

Direct communication between visits. If something comes up between appointments — a reaction, a question, a missed dose — you have direct access to Dr. Sivam, not a phone tree.

Common Questions From Peanut Allergy Families

Is OIT safe? OIT is a medical treatment that carries real risks, including the possibility of allergic reactions during dosing. That is why it must be done under the supervision of a board-certified allergist with proper protocols in place. At Inspire, we take safety seriously at every phase of treatment.

How long does peanut OIT take? The build-up phase typically takes several months. Maintenance is ongoing. The exact timeline depends on your child's starting point, how they respond to dose increases, and the treatment goals we set together.

Does my child need to stay on OIT forever? Most patients require continued daily dosing to maintain their protection. Some patients achieve a level of tolerance that allows them to consume peanut freely, but this varies. We discuss realistic expectations with every family before starting treatment.

What if my child has other food allergies too? OIT is food-specific, so treating multiple allergens requires separate protocols. We discuss your child's full allergy picture at the initial evaluation and help you prioritize.

Does insurance cover OIT? Inspire Allergy & Asthma operates as a direct care, out-of-network practice. OIT is priced transparently and billed through our membership model. Biologics and some medications may still go through insurance when appropriate.
