If you’re constantly fighting congestion, struggling to catch your breath, or watching your child’s skin flare with irritation — you know how exhausting allergies and asthma can be.

All new patients will be scheduled for an initial consultation appointment with Dr. Sivam prior to enrolling in membership. At the time of the initial consultation, Dr. Sivam can assist in determining if membership is right for you.

Please note, as a direct care practice, Dr. Sivam is not in network with any insurance. Patients receive an invoice after the appointment and charges are placed to the payment on file. We accept all major credit cards, check, and bank transfer.

Membership program: Membership enables consistent, high-quality personalized allergy and asthma care when you need it. Benefits of membership include unlimited appointments, priority scheduling, complimentary prior authorization for biologics and medications, and unlimited direct communication during business hours with Dr. Sivam via text or phone. 

Non-membership: Patients who do not opt for membership will pay for visits and services a la carte. Pricing for this option is listed below. This is recommended for those who need only 1-2 visits per year and minimal between-visit care.

Pricing

  • $275

    • A comprehensive allergy-focused intake and physical examination

    • Detailed review of your past allergy records

    • Ordering of lab tests and imaging, if medically necessary

    • Prescriptions for new medications, when appropriate

    • Review and interpretation of any previous allergy testing

    • Discussion of possible triggers and lifestyle factors contributing to symptoms

    • Personalized treatment plan tailored to your specific concerns

    • Time for your questions to be answered thoroughly and clearly

    • Communication with Dr. Sivam regarding your care plan and medications is included for two weeks following your appointment. After this time, additional questions may incur a separate communication fee or you may be advised to schedule a follow-up visit.

    Dr. Sivam will help you decide if allergy testing or pulmonary function testing is necessary and beneficial for your treatment plan after a thorough intake of your history. Returning patients of Dr. Sivam receive a courtesy discount on initial visit fees.

     

    *We require 48 hours' notice for cancellation of a new patient consultation. In the event that a new patient consultation is cancelled less than 48 hours before the appointment or you are a no show,  a $75 late cancellation/no show fee will be charged to the credit card on file.

    Our membership program is ideal for individuals with ongoing allergic conditions such as asthma, food allergies (for patients NOT enrolling in the OIT program), eczema, or chronic hives. It offers a high level of access, support, and continuity of care tailored to your needs. Discounts offered for multiple months paid in advance. Generally, if you are on a biologic, we recommend the membership plan.

     

    Membership includes:

    • Priority Scheduling: Enjoy convenient visits as often as medically necessary.

    • Patient Portal Access: Securely view test results, medical records, request prescription refills, and communicate directly with our office.

    • Direct Communication: Contact Dr. Sivam directly via patient portal for timely support and guidance.

    • Expedited Prior Authorizations: We work quickly with your insurance company to help obtain necessary approvals for medications, imaging, or testing.

    Please note: Membership does not include the cost of therapeutics such as epinephrine auto-injectors (e.g., EpiPen), inhalers, oral or topical medications, or biologics like Xolair, Dupixent, etc. These may be prescribed or recommended based on your condition, but their cost is separate and paid directly to the pharmacy or supplier. This also does not include the cost of allergen immunotherapy (allergy shots or drops). This does not include the cost of oral immunotherapy for foods.

    • $400

      (​Up to 40 allergens – including pollens, dust mites, pets, molds):

      We offer testing for 40 common environmental allergens, including pollens, dust mites, pet dander, and molds. This testing helps identify triggers that may be causing your allergy symptoms, so we can create a personalized treatment plan to help you feel better.

    • $10/food test

      We offer food allergy testing using skin prick tests, with charges applied per prick test. This testing is designed to identify IgE-mediated food allergies based on your allergic history. Please note that food allergy testing is not recommended for symptoms related to food intolerances or sensitivities, such as gastrointestinal distress, migraines, or other non-allergic reactions.

      A Follow-Up Visit is a crucial part of your care and treatment journey. After your initial consultation or procedure, our team schedules a follow-up to ensure that you are progressing well, to discuss any questions or concerns, and to make any necessary adjustments to your treatment plan. This "a la carte" follow-up visit is recommended if you anticipate 1-2 visits per year.

       

      This visit allows us to:

      • Monitor progress: Ensure that your treatment is working effectively and address any potential issues.

      • Provide ongoing care: Adjust medications, therapies, or lifestyle recommendations based on your current health status.

      • Answer questions: Clarify any doubts you might have or provide additional information about your recovery or next steps.

      • Offer support: Our team is here to help you throughout your recovery process and beyond, ensuring you feel confident and supported.

      • Communication with Dr. Sivam regarding your care plan and medications is included for two weeks following your appointment. After this time, additional questions may incur a separate communication fee or you may be advised to schedule a follow-up visit.

       

      Follow-up visits are typically scheduled 4 weeks after your initial consultation, sometimes sooner depending on what condition we are treating. Your health and comfort are our top priorities, and we are here to guide you every step of the way.

      An oral food challenge is a closely monitored visit where we safely test your response to a specific food. You’ll be cared for every step of the way, with medical supervision and emergency precautions in place. The process usually takes up to three hours and helps us confirm whether a true food allergy is present—or rule it out with confidence.

    • $900

      Patch testing helps identify topical substances like fragrances or metals that cause delayed skin allergies, often responsible for chronic rashes or eczema. Small amounts of common allergens are placed on the back and worn for several days, followed by brief in-office checks over the week to see how your skin reacts. This allows us to pinpoint triggers and create a personalized plan to help your skin heal.

    • $300

      If you notice a sudden worsening of your allergy or asthma symptoms, or if you experience any unexpected or concerning reactions, please contact us as soon as possible. Prompt evaluation can help prevent complications and keep you safe. Examples can include asthma exacerbation or an eczema flare. (Urgent visits are included in the membership plan.)

       

      For any severe or life-threatening symptoms, please call 911 or visit the nearest emergency room immediately.

    • $50

      To better understand your breathing and evaluate conditions like asthma, we may recommend simple, non-invasive tests such as spirometry (also known as pulmonary function tests.) Spirometry measures how well your lungs are working by assessing airflow and lung function.​ This test provides valuable information that helps us tailor your treatment plan for better breathing and overall respiratory health.

    • $75 Initial prior authorization submission
      $75 Appeal if authorization is denied
      $125 If peer-to-peer review with insurance physician is required

      Some medications and biologic therapies require prior authorization from your insurance company before they will approve coverage. Unfortunately, this process often involves significant back-and-forth communication with insurers, submitting detailed clinical documentation, responding to additional requests, and sometimes completing multiple rounds of appeals or peer-to-peer reviews.

      At Inspire Allergy & Asthma, we are committed to advocating for our patients when insurance approval is required. However, managing prior authorizations requires dedicated physician and staff time coordinating with insurance companies. To keep our pricing transparent and sustainable, fees apply when our team completes this work on your behalf. While we work hard to support patients through this process, insurance companies ultimately make the final decision regarding medication approval and coverage.

      This process is commonly required for biologic medications used to treat severe asthma, eczema, chronic sinus disease with nasal polyps, and other allergic conditions, such as Dupixent®, Xolair®, Nucala®, Fasenra®, or Tezspire®.

      Patients receiving biologic therapies often find that our membership model provides the best overall value, as prior authorization submissions and ongoing biologic coordination are included in membership services.

    • $300/hour

      Phone or text support after the two-week communication period following your visit will be charged in 10-minute increments, prorated to $300/hour. A follow-up visit may be recommended depending on the complexity of the question.

    • $10 per page per form

      We know that managing allergies and asthma often comes with plenty of forms! Our team is happy to help with school, camp, or action plan paperwork related to your care. Because these forms take time and attention to complete accurately, please allow a few business days for processing.

    • $50 / injection

      Biologic treatments are used for patients with severe asthma, food allergies, chronic hives, or nasal polyps that don't respond well to standard therapies. If prescribed, you can choose to receive these targeted injections at our clinic under medical supervision to ensure safety and effectiveness.

    • Cost dependent on service

      In addition to our core allergy and asthma services, we offer select evaluations and care options on an individualized basis. These may include areas such as medication or vaccine allergy evaluations, mast cell–related symptom assessments, immune system evaluations, or other specialized consultations. Services are considered thoughtfully based on your symptoms, goals, and clinical history, and are discussed together prior to scheduling to ensure the right approach for your care.
