Dr. Sivam will help you decide if testing is necessary and beneficial for your treatment plan after a thorough intake of your history. One month of complimentary membership for those who enroll in membership at the initial visit. Returning patients of Dr. Sivam receive a courtesy discount on initial visit fees.
*A deposit of $50 is due at the time you schedule a new patient consultation. This deposit will be applied to the cost of the consultation on the day of your visit. We require 48 hours' notice for cancellation of a new patient consultation. In the event that a new patient consultation is cancelled less than 48 hours before the appointment or you are a no show, this deposit will serve as the late cancellation/no show fee.

Our membership program is ideal for individuals with ongoing allergic conditions such as asthma, food allergies (for patients NOT enrolling in the OIT program), eczema, or chronic hives. It offers a high level of access, support, and continuity of care tailored to your needs. Discounts offered for multiple months paid in advance.

Please note: Membership does not include the cost of therapeutics such as epinephrine auto-injectors (e.g., EpiPen), inhalers, oral or topical medications, or biologics like Xolair, Dupixent, etc. These may be prescribed or recommended based on your condition, but their cost is separate and paid directly to the pharmacy or supplier. This also does not include the cost of allergen immunotherapy (allergy shots or drops). This does not include the cost of oral immunotherapy for foods.

(Up to 40 allergens – including pollens, dust mites, pets, molds):
We offer testing for 40 common environmental allergens, including pollens, dust mites, pet dander, and molds. This testing helps identify triggers that may be causing your allergy symptoms, so we can create a personalized treatment plan to help you feel better.

We offer food allergy testing using skin prick tests, with charges applied per prick test. This testing is designed to identify IgE-mediated food allergies based on your allergic history. Please note that food allergy testing is not recommended for symptoms related to food intolerances or sensitivities, such as gastrointestinal distress, migraines, or other non-allergic reactions.

A Follow-Up Visit is a crucial part of your care and treatment journey. After your initial consultation or procedure, our team schedules a follow-up to ensure that you are progressing well, to discuss any questions or concerns, and to make any necessary adjustments to your treatment plan. This "a la carte" follow-up visit is recommended if you anticipate 1-2 visits per year.

Follow-up visits are typically scheduled 4 weeks after your initial consultation, sometimes sooner depending on what condition we are treating. Your health and comfort are our top priorities, and we are here to guide you every step of the way.

An oral food challenge is a closely monitored visit where we safely test your response to a specific food. You’ll be cared for every step of the way, with medical supervision and emergency precautions in place. The process usually takes up to three hours and helps us confirm whether a true food allergy is present—or rule it out with confidence.

If you notice a sudden worsening of your allergy or asthma symptoms, or if you experience any unexpected or concerning reactions, please contact us as soon as possible. Prompt evaluation can help prevent complications and keep you safe. Examples can include asthma exacerbation or an eczema flare.
For any severe or life-threatening symptoms, please call 911 or visit the nearest emergency room immediately.

To better understand your breathing and evaluate conditions like asthma, we may recommend simple, non-invasive tests such as spirometry (also known as pulmonary function tests.) Spirometry measures how well your lungs are working by assessing airflow and lung function. This test provides valuable information that helps us tailor your treatment plan for better breathing and overall respiratory health.
$10 per page per form
We know that managing allergies and asthma often comes with plenty of forms! Our team is happy to help with school, camp, or action plan paperwork related to your care. Because these forms take time and attention to complete accurately, please allow a few business days for processing.

Biologic treatments are used for patients with severe asthma, food allergies, chronic hives, or nasal polyps that don't respond well to standard therapies. If prescribed, you can choose to receive these targeted injections at our clinic under medical supervision to ensure safety and effectiveness.
