Oral Immunotherapy for Food Allergies in Carmel, Indiana: Is It Right for Your Family?

If your child, or you, has a food allergy, you already know what the day to day looks like. Reading every label twice. Asking the same questions at every restaurant. Packing a safe snack just in case. Carrying epinephrine everywhere, and still feeling the low hum of worry that never quite turns off.

Avoidance has been the standard approach for decades, and it matters. But for many families, avoidance alone does not feel like enough. It reduces risk without ever addressing it, and it asks a lot of a child, a classroom, a birthday party, a life.

That is where oral immunotherapy comes in.

**What It's Like to Live With a Food Allergy Every Day**

Food allergy does not stay contained to mealtimes. It shows up in school lunch decisions, sports team snacks, sleepovers, holidays, and travel. Families build routines around it without even noticing: the extra label check, the epinephrine bag by the door, the quiet calculation before saying yes to a party invitation.

Most families manage this well. But managing it is not the same as feeling at ease with it. Many parents tell us they are tired of hoping nothing slips through, rather than having something that actively lowers the risk.

**Why Avoidance Alone Doesn't Feel Like Enough**

Avoidance asks a family to control an uncontrollable amount of variables: ingredient lists, cross-contact in a kitchen, a well-meaning grandparent offering "just a bite." Even the most careful family cannot avoid every possible exposure forever.

What avoidance cannot do is change how the body responds if an exposure happens. That is a different kind of protection, and it requires a different kind of treatment.

**What Most Families Are Never Told About Their Options**

Many patients come to our practice having never been told that food allergy can be actively treated. They assume avoidance and an epinephrine prescription are the only tools available, because that is often what they have been told by others.

Oral immunotherapy has been studied for years and is offered by a growing number of allergists, but it still is not widely explained to families. Part of our role is simply making sure you know it exists, understand how it works, and can decide for yourself whether it fits your family.

**What Oral Immunotherapy Actually Is**

Oral immunotherapy, or OIT, is a medically supervised treatment that helps the immune system become less reactive to a specific food allergen over time. Instead of avoiding the allergen completely, a patient consumes a small, carefully measured amount of it, starting at a very low dose and increasing gradually under the guidance of a board-certified allergist.

The goal of OIT is desensitization. This means raising the threshold at which a reaction occurs, so an accidental exposure, a crumb, a trace amount, a "may contain" label, is far less likely to cause a serious reaction. For some patients, OIT can eventually allow more freedom with the food itself, though that outcome varies and is not the primary goal for every family.

OIT does not erase a food allergy. It changes the relationship between the body and the allergen, and for many families, that shift changes daily life in a meaningful way.

**What OIT Looks Like at Inspire Allergy & Asthma**

OIT follows a structured, phase-based process that we tailor to each patient.

**Evaluation and preparation.** Before treatment begins, we conduct a thorough food allergy evaluation. This includes a detailed history, prior testing results, and a conversation about your goals and any other health conditions, such as asthma or eczema, that could affect the plan.

**Initial dosing.** The first dose is given in our office under close observation. We start with an extremely small amount of the allergen and watch carefully for any response. This visit runs longer than a typical appointment, on purpose.

**Build-up dosing.** Over the following weeks and months, the dose increases in small, planned steps. Build-up visits happen in our office, usually every two to four weeks, with daily dosing at home in between. Consistency at home is what drives the process forward.

**Maintenance.** Once a patient reaches their target maintenance dose, they continue taking it daily at home, with regular follow-up visits to monitor progress and adjust the plan as needed.

At every phase, you work directly with Dr. Sivam and our team. As a direct-care practice, we do not route OIT visits through a call center or hand you off between providers. Every dose change and every question goes through your allergist, with transparent pricing and direct communication between visits.

**Is OIT Right for Your Family?**

OIT is not the right fit for every patient, and part of our job is helping you figure out whether it makes sense for yours. Good candidates generally include children and adults with a confirmed IgE-mediated food allergy, patients who are medically stable, and families who are ready for the commitment of daily dosing and regular visits.

The best way to know is a conversation. We would rather walk through your child's specific history and answer your questions directly than have you guess from a website.

**Common Questions From Families**

**Is OIT safe?**

OIT is a medical treatment that carries real risks, including the possibility of an allergic reaction during dosing. That is why it must be done under the close supervision of a board-certified allergist with proper protocols in place. We take safety seriously at every phase of treatment.

**How long does OIT take before my child is protected?**

The build-up phase typically takes several months, and maintenance continues after that. The exact timeline depends on your child's starting point, how they respond to dose increases, and the goals we set together.

**Will my child be able to eat the food freely afterward?**

Some patients reach a level of tolerance that allows more freedom with the food, but this varies from patient to patient and is not the primary goal for everyone. The main goal of OIT is raising the threshold at which a reaction occurs, so accidental exposures are less likely to cause a serious reaction. We discuss realistic expectations with every family before starting treatment.
